Utah defends its hands-off health insurance exchange plan | The ...

But it proved easier than state officials ever imagined.

Examining exchange plans

In letters to the U.S. Department of Health and Human Services last week, several advocacy groups complained about the lack of clarity and transparency surrounding Utah?s plans for Avenue H. Among them: a physicians group, federally certified community health clinics, AARP Utah, Voices for Utah Children and the Utah Health Policy Project.

The Salt Lake Tribune obtained parts of Utah?s exchange blueprint through a records request. They make promises, say advocates, but don?t say how the state will live up to them.

?There?s so much in there that?s theoretical,? said Judi Hilman, executive director of the Utah Health Policy Project. ?If I were the feds, I would agree to a federally facilitated exchange or give Utah conditional approval subject to our meeting certain requirements and deadlines.?

That?s because hospitals, looking to get paid for treating uninsured patients, did the heavy lifting. They filled out and submitted papers on behalf of patients and, in some cases, paid their monthly premiums, no strings attached.

"People who have a financial interest in making things work will find a way to do it, and do it quickly," said Utah Gov. Gary Herbert?s health adviser, Norm Thurston, citing the experience as proof that Utah?s hands-off, private-market approach to another Affordable Care Act (ACA) requirement ? health exchanges ? will work.

Exchanges, online insurance marketplaces, are designed to be a "no wrong door" portal to health coverage where consumers can compare plans, see if they?re eligible for federal subsidies to purchase them, or enroll in low-income programs such as Medicaid.

"The idea is you can enter this door and walk out with coverage," said Lincoln Nehring, a health policy analyst at Voices for Utah Children.

Utah already has a "shop" exchange for small businesses, Avenue H, which the governor has asked the U.S. Department of Health and Human Services (HHS) to declare as good enough for Obamacare.

Herbert promises to open it to individuals and larger employers and to enforce new insurance price controls and consumer protections. But he argues the customer outreach and hand-holding required to help people navigate insurance options are a waste of money and unrealistic.

His proposal is being viewed as a test case of just how much leeway the Obama administration is willing to give states in running their own exchanges.

HHS will not comment on the negotiations, but Thurston said sticking points are mostly on small, technical issues.

It?s the details that worry consumer advocates, who are pushing for greater clarity on Utah?s plan and fear Herbert?s vision for Avenue H falls woefully short.

Of top concern, they say, is Utah?s poor track record in linking eligible Utahns with health safety nets.

The state ranked 50th in enrolling eligible children in Medicaid and the Children?s Health Insurance Program (CHIP) in 2010, said Nehring, noting that 80 percent of the state?s 102,400 children are eligible for these programs.

"Utah families are in particular need," he said, "of the ACA?s vision of an exchange that will connect [them] with all their health coverage options."

Under the ACA, those shopping on an exchange must first be screened to see if they?re entitled to Medicaid or CHIP. They are also screened for federal tax credits to apply toward the purchase of their coverage.

States are supposed to hire navigators to help people pick plans best suited to their needs. The rules say they have to provide interpretive services, aides for people with disabilities, and publish "culturally and linguistically appropriate outreach" materials.

But Herbert?s advisers argue these services are widely available in the community.

(AP) ? They don't have demands, but they're loyal to the cause and are always on the front lines of the fight. They run with protesters, lap up shots from water cannons, bark at police in riot gear and sometimes even bite officers.

Stray dogs are truly Man's Best Friend for thousands of students and workers who demonstrate and clash with police nearly every day to press demands for education improvements, redistribution of Chile's wealth and environmental protections. As the protests become fixtures in this modernizing capital, normally unnoticed street dogs have become stars in their own right, with the Facebook fan pages and fawning media coverage to prove it.

"Blacky," a mutt adopted by young protesters, has become the most visible mascot, with rival fan pages totaling more than 7,000 subscribers or "likes." Blacky's admirers constantly upload pictures of him, many showing the mutt with a checkered kaffiyeh around his neck symbolizing the Palestinian resistance movement, dodging tear gas or growling at baton-wielding officers.

"Dogs are super loyal. They stand with the people and I think they support the students," said Catalina Echenique, 17, who is planning to study psychiatry.

Free-roaming dogs number in the millions in Chile in a situation the nation's Humane Society has called alarming. Dog owners rarely spay or neuter their pets, and commonly leave them outside when they go to work in the morning. Many roam the streets all day.

Dogs lurk around the presidential palace, take naps in parks and always seem in search of a bite to eat or the next protest.

While strays are feared in countries such as India, where tens of millions of street dogs have a reputation for biting people and spreading rabies, Chileans often feed and take care of strays. Protesters, for one, are glad to have the dogs on their side of the fight.

Students have been hitting the streets for more than a year and a half demanding overhauls to a school system that's been privatized since the 1973-90 dictatorship of Gen. Augusto Pinochet. Protesters say families must struggle with underperforming public schools, expensive private universities and education loans at impossible interest rates.

Two military officers in impeccable white uniforms walked out of a subway station recently as two blackened mutts followed them ahead of a crowd of young protesters who booed and shouted insults.

More dogs followed the sounds of sirens ? and the promise of a water jet some blocks away. Police fired tear gas and the hounds ran to chew on the canisters. From a plume of smoke, Blacky dashed out, this time wearing an orange bandanna.

A mass of students and hooded members of anarchist groups loitering at nearby parks flooded into the streets for yet another confrontation.

Meanwhile, Echenique sat in a circle with other students, a stray napping next to them while they prepared to clash with police.

"With a good education we can generate conscience to protect animals," she said.

Despite the propensity of dog packs to join protesters, they're not at constant war with the police.

Just a few blocks from the recent confrontation, police and pooches appeared to be enjoying a peaceful timeout. One stray snoozed under the noon sun next to a traffic officer at a busy intersection, while another quietly napped in the shade cast by paintings propped on artist easels in Santiago's main square, the Plaza de Armas.

"I see the ritual everyday: police dogs patrolling the streets and strays watching over their territory," said Mario Guitierrez, a 52-year-old artist at the square who plans to make the protest dogs the subject of his next work.

"They meet, they stare, and it seems like the police dogs get scared. The street dogs are brave!"

Police officer Eduardo Basaez of the department's canine training unit strolled by with Lola, his 7-year-old German shepherd. They have both been part of street clashes, but Basaez said police dogs and horses are being used less these days to keep them safe.

"Dogs go to the protests because of a pack instinct. They play with the water jets, they're happy and don't know what's going on," Basaez said. "I'm a dog lover and I feel sorry for the street dogs. I live in an apartment but if I had a tract of land I would take them all home with me."

FBI question Benghazi consulate attack suspect

TUNIS, Tunisia (AP) ? After months of asking, agents from the FBI questioned the only known suspect in the Sept. 11 attack on the U.S. consulate in Benghazi that killed four diplomats, the suspect's Tunisian lawyer told The Associated Press Saturday.

Ali Harzi, a Tunisian, was detained in Turkey and extradited to Tunisia in October where authorities have said he is "strongly suspected" of being involved in the attack.

His lawyer, Anwar Oued-Ali, added that Harzi was also questioned about an attack on the U.S. embassy in Tunisia, a few days later, suggesting the American authorities are looking into if there is a connection between the two attacks.

The Sept. 11 assault by armed men in the Libyan city of Benghazi killed U.S. Ambassador Chris Stephens and three other American diplomats. Members of an Islamist militia, Ansar al-Sharia are suspected in the strike, but there has been little progress in the Libya-based investigation into the attack.

A few days later, a mob attacked the U.S. embassy in Tunis, destroying property and an American school in the area, resulting in four deaths. The attack was believed to be instigated by a local group also called Ansar al-Sharia, but it is unclear if it is connected to the Libyan organization.

In early November, Republican senators Lindsey Graham and Saxby Chambliss announced that Tunisia had agreed to allow the FBI to interview Harzi, but it took another month and a half to organize the interview due to legal questions over any infringements on Tunisian sovereignty.

In the end, three FBI investigators using a Moroccan translator posed questions to Harzi for three hours through the Tunisian judge presiding over the case.

Harzi's defense team was not allowed to attend the questioning on the grounds that he was being interviewed as a "witness" rather than a defendant.

Harzi is being charged by the Tunisians for "membership in a terrorist organization." Harzi denies the charges.

Goldoak Mountains
Far to the North-East of the Silkleaf Mesa is a chain of old, rounded mountains. Nestled within one particularly fertile valley is a grove of trees with leaves as yellow as gold. It is unsure as to whether or not this is a natural phenomena, but the citizens of the nearby Daedalus don?t seem to mind. They also don?t mind sharing their home with the untame Illimeur of the forests or the friendly wrendels wandering the orchards. They do, however, hunt the gryphons that nest near the peaks of their mountains.

The Isle of Clouds
It is not often that an island migrates on its own, but this tiny one seems to have decided that spending its life in one place is far too dull. It drifts about the seas as it pleases, a skirt of clouds clinging to it and trailing in its wake. Common birds nest in this region, as well as the elusive Quel. It is even rumored that a family of Doola roam the dense forests of its single mountain.

Silkleaf Mesa
To the East of the Wild Plains a lone, flattened mountain stands. While the earth atop it is mostly barren and dry, a massive swath of brambles marks Hellquill Nest. South of this loathsome mess of thorns is the Jadepyre Post.

Tiny lizards, buzzards, and a medley of other desert-like critters wrestle with the heat here, as well as basilisks, wyverns, and lefor.

The Wild Plains
To the West of Silkleaf Mesa lies a vast, mostly uncharted area. Seas of grasses stretch as far as the eye can see, and massive beasts roam the land. There is nearly no protection from the scorching rays of the sun in this grassland.

The Shore of Flames
Before the Ashriders came to be, dragons were uncontrollable monstrosities of destruction. As a result, a force of soldiers were sent by an ancient king to their nest, slaughtering as many as they could. Many brave men died that day, but so did many fearsome dragons.

However, an unforeseen consequence sprung up as a result. The dragons bled liquid flames, and thus the shores of their seaside cave were set ablaze. Decades have past, but the ever-burning blood of the dragons has not dimmed in the slightest.

The Spirit Basin
The earth here bleeds an unnatural glow; some magic as thick as blood that seeps up through the ground in wells and streams. Very little lives here, but what does is twisted and bizarre.

The Voidcrest
Far to the North of Islithika lies a dark, shadowy mass. The world crumbles at its edges, and dark tendrils of some unknown magic snake across the hills around it and reach into the sky. Void monstrosities slither out of it, but mortal has ever entered the Voidcrest and returned.

THURSDAY, Dec. 20 (HealthDay News) ? Researchers who discovered that some supposedly random gene mutations are not quite so random after all say their findings offer clues to the causes of autism and other disorders.

The international team of scientists sequenced the complete genomes of identical twins with autism and their parents and found that the DNA sequence in some regions of the human genome is quite unstable and can mutate 10 times more frequently than the rest of the genome.

Genes that are linked to autism and a number of other disorders have a particularly strong tendency to mutate, according to the study published Dec. 21 in the journal Cell.

?Our findings provide some insights into the underlying basis of autism ? that, surprisingly, the genome is not shy about tinkering with its important genes. To the contrary, disease-causing genes tend to be hypermutable,? principal investigator Jonathan Sebat, a professor of psychiatry and cellular and molecule medicine at the University of California, San Diego, said in a university news release.

An average of 60 mutations were detected in each child in the study, according to the report.

?The total number of mutations that we found was not surprising, it?s exactly what we would expect based on the normal human mutation rate,? said Sebat, chief of the Beyster Center for Molecular Genomics of Neuropsychiatric Diseases at UCSD.

However, he and his colleagues were surprised to find that mutations tended to cluster in certain regions of the genome.

?We plan to focus on these mutation hotspots in our future studies,? Sebat said. ?Sequencing these regions in larger numbers of patients could enable us to identify more of the genetic risk factors for autism.?

(AP) ? The body of Sen. Daniel Inouye of Hawaii arrived Thursday at the U.S. Capitol, where former colleagues and the public had a rare opportunity to pay respects to a man who served his country as a soldier in World War II and was a U.S. senator for 50 years.

Inouye was to lie in state throughout the day in the Capitol Rotunda. The tribute demonstrated the respect and good will that lawmakers in both parties had toward the soft-spoken but powerful Democratic chairman of the Senate Appropriations Committee. Inouye died Monday from respiratory complications. He was 88.

Only 31 people have lain in the Capitol rotunda. The last was former President Gerald R. Ford nearly six years ago. The last senator so honored was Democrat Hubert Humphrey of Minnesota in 1978.

"He was an institution unto himself, and he deserves to spend one more day in this institution, to which he dedicated his life's work," said Senate Majority Leader Harry Reid, D-Nev.

Before Inouye made a mark as a politician, he did so as a war hero who lost his right arm while leading his platoon into battle on a ridge in Italy. He was awarded the Medal of Honor, the nation's highest military honor.

Following Hawaii's statehood in 1959, Inouye became its first congressman. He won election to the Senate in 1962. He was the first Japanese-American elected to both the House and Senate and was serving his ninth term in the Senate when he died. As a legislator, his specialty was steering federal money to his home state to develop the kinds of roads, schools and housing other Americans had on the mainland.

After a morning service and the ceremonial laying of wreaths, a viewing that would be open to the public was planned.

Inouye's body will be escorted Friday to the Washington National Cathedral and will be returned to Hawaii on Saturday.

'X Factor' Jahmene Douglas signs record deal with Sony

Jahmene Douglas has signed a record deal.

In the aftermath of Hurricane Sandy, big changes are going to be made in the coming year or two.? Flood maps are being revised to reflect the weather risk to not only low lying areas, but areas that are prone to flood during another potential storm surge.? The devestating results in New Jersey is going to see an insurance reform that will take years to reform.

Although flood maps will not be revised until sometime in 2014, you can expect Flood Insurance to go up across the board where it is required.? Once the revisions come out, and those areas newly designated as ?flood-hazard areas?, the new policies issued will begin at subsidized rates and the premiums will increase by 20% a year for five years.

Any home with a mortgage sponsored by the government such as Fannie Mae, Freddie Mac, Veterans Assistance, FHA, etc, will be required to have flood insurance.? Homes without it will be given 45 days notice to place a flood policy on the home, otherwise the mortgage companies will force place a very pricey policy for you.? According to FEMA, the average flood policy costs $625 a year where homes in highest-risk areas cost more then $3,000.? This cost is expected to double.

So what does this mean to Hoboken, Jersey City, and all of the coastal urban areas that have Homeowners Associations in charge of their insurance premiums.? Expect an increase in HOA dues as a result of higher flood premiums.? This isn?t a matter of if, it?s a matter of when.

Now when homeowners are looking at areas like Hackensack versus Clifton, the cost of insurance will now come into play when breaking down the monthly payment and risk assessment.? Now, will another storm like Sandy happen again in NJ?? History says it is very possible since the last 2 years have brought a storm, but all we can do is live our life and hope that it doesn?t.? Homeowners will need to pay extra special attention to their policy especially if they are a primary homeowner or a second home / investor owner.? Policies are written very different.? Remember the terms DP1 and DP2.? Ask what the difference in coverage is.? Also, on primary homes, the coverage is based on replacement cost.? On second homes and investment property, it could be based on depreciable value, a massive difference.? Don?t be fooled or don?t accept just the cheapest policy.? Be sure you understand what your policy covers, know your deductable, and find out what your coverage is if the storm is a hurricane as opposed to just a tropical storm or torrential rain.

The increase in flood insurance could play into the housing crisis, especially in Jersey Shore areas where it would simply be too expensive to maintain a home there with the added cost.? Although coverage for say 100 unit building is way more then a single family home policy, it is still less then 100 single family homes and each unit will pay their share of the increase so the cost effect will be less on buildings who are managed by associations.

As we prepare for higher premiums, we must also take into account that premiums can be mitigated when rebuilding or new construction.? Being compliant with the new revised flood maps issued by FEMA, your flood policy will drastically be lower then those homes that are only compliant and grandfathered into the former building code.? My guess is that homes along flood prone areas will need to be raised 4 feet above where the stand today.? This can be established using extra dirt on your property to build up your home or also elevated on pilings which would give you a crawl space under your home.? There are actually very beatiful designs for homes with this type of elevation in Florida, the Carolinas, etc.? NJ will need to adapt to those type of plans if they want to live along the coast and mitigate insurance costs.

Angry with Instagram? These 'invisible' data brokers sell your privacy every day

Instagram's abrupt change of terms this week created a predictable Internet chatter bomb, as Web users erupted in anger that the firm might violate their privacy and property rights. Sadly, there is no such outrage at companies which buy and sell our privacy as their business model ? and much less interest in promising efforts to rein them in.

What do they know about you and when did they buy it? The Federal Trade Commission this week joined a series of other agencies, groups, and elected officials now banging on the door of the nation's largest data collection firms, demanding to know just how much of U.S. consumers' lives are tracked by these firms.?

Firms you've probably never heard of with names like Axciom, Intellius, Datalogix, and RapLeaf acquire, store, and sell hundreds of pieces of information about you to voracious marketers hungry for an advertising edge.? On Tuesday, the Federal Trade Commission announced it had opened an inquiry into the business practices of those four firm and five others like them. It has issued administrative subpoenas to those firms, asking for a detailed accounting of what they know about American consumers, and what they do with it. There's big business in following you around and predicting what you might buy next, but the mysterious shroud these companies operate behind frustrates privacy advocates, who say consumers would be frustrated, too if they really knew what was going on.

"Many data brokerage companies are engaging in business practices without consumer knowledge or consent ? including the collection, use, and sale of personal information about the American public," said Rep. Ed Markey, D-Massachusetts, in a statement supporting the FTC?s action.

While most fascination with the business has been with the spookiest practices???location tracking through cellphones, or Web click-path monitoring, it's important to note that firms like Axciom marry both online and offline data points. The profile data is then sold to companies that want to engage in more and more targeted marketing. That's how Target can learn that a teenager is pregnant before her father does, for example.

Earlier this year, the Federal Trade Commission issued a report criticizing the "invisibility" of the data brokers, and on Tuesday it officially joined a host of others looking into the industry: In July, Markey and Rep. Joe Barton, R-Texas, launched their own investigation into data brokers. In October, Rep. John D. Rockefeller, D-West Virginia, said he planned wanted a Congressional inquiry, too.

What makes data brokers different from other firms that know a lot about consumers, like credit report agencies?

Consumers have plenty of complaints about Equifax, Experian and TransUnion, which store and sell financial data, and play a big role in borrowers' ability to get loans (There were 30,203 complaints against them filed in 2011, the FTC?s annual report says) . But since 2001, consumers have been entitled to see what credit reporting agencies know about them, for free, and have legal rights to demand mistakes be fixed.

Data brokers operate under entirely different rules.? Consumers have no right to know what data brokers know about them. Most don't even know who these companies are, let alone what information they've gathered, and what?s happening to it.

"It is appalling that these companies can still freely use personal information such as a user?s full name, home address, social security number, telephone number, and even birth date at their discretion," said Barton said in a statement after the FTC's announcement. "It is time that Congress carefully considers this industry and the potential dangers that exist."

Data brokers have repeatedly defended themselves by arguing that targeted advertising is a boon for consumers, who enjoy more relevant advertisements as the result of highly sophisticated data collection and analysis.

"The incredible growth of e-commerce ? with online spending on Cyber Monday soaring to $1.46 billion this year ? is strong evidence that consumers are delighted with the results of data-driven marketing," said Jerry Cerasale, executive of the Direct Marketing Association, in a statement issued after the FTC announcement. "E-commerce is exploding because responsible data-driven marketing is meeting consumers' needs."

I don't know about you, but I definitely don't remember thinking, "Hey, I think I'll buy my sister an extra present this year," because some company had stalked me around the Internet.

In fact, with all the arguing about massive data collection and privacy, I feel like I see more ads than ever that miss the mark. Case in point: I'm a musician, and recently, I shopped around online for new drums. I purchased them rather quickly. For weeks after, I felt like music stores had hijacked my computer, shoving ads for the exact drum kits I'd considered over nearly every website I visited. ?

The technique is called ?retargeting,? and the ads are designed to follow you around the Web. I get the concept: If you are shopping for something, you are probably feeling transactional, and that's exactly when the right ad might push you over the edge and into a company's revenue stream.

Close, but no cigar, music stores. Wait a minute: it?s not close at all! Has any advertisement in history of all media been *less* effective than an ad shown to a consumer who has just purchased that very thing? Think about it: You buy a BMW, and then all TV shows you watch are interrupted with BMW ads. How dumb is that? Random ads would be much more valuable than these so-called retargeted ads.

But even retargeting barely begins to scratch the surface of data-driven advertising ? it involves simple use of old-fashioned cookies. The capabilities of marketing firms to know how old we, where we live, how much we make, when we are most likely to shop, who our friends might be?the spookiness potential is endless.

We can't count on clumsiness to protect us from the potential privacy problems caused by unfettered, unregulated data collection and sale. The FTC has advised Congress to pass new laws regulating this industry, and many of the agency's proposals make sense, and deserve support as loud as the fury Instagram felt this week.

The first, and most obvious, element of the FTC's plan: Data brokers should be required to tell consumers what they know about them, for free, the same way credit bureaus now make annual disclosures to consumers.

"To address the invisibility of, and consumers? lack of control over, data brokers? collection and use of consumer information, the Commission supports targeted legislation ? similar to that contained in several of the data security bills introduced in the 112th Congress ? that would provide consumers with access to information about them held by a data broker," the FTC said.

Currently Axciom sells such reports to consumers for $5. What's worse, consumers have to actually mail ??as in, physically mail ? a personal check to the firm to pay for the report. For such a high-tech database operation, that's antagonistically old-school.?

And naturally, few consumers take this option because they've never heard of Axciom ?? it's even less likely they've heard of smaller data brokers. So the FTC also recommends these firms be forced to create a central clearinghouse for consumers can find out about such data collection companies and make their queries in one place.

Disclosure is start, but only a start. We learned this week that only one in five consumers review their credit report each year, according to the Consumer Financial Protection Bureau ?? and that's with direct economic incentive to do so. Credit report mistakes cost consumers money. You can assume far fewer than 20 percent would review their data broker reports. So it's critical that Congress and the FTC go much farther than disclosure and impose tight data security standards on firms, make it easy for consumers to avoid tracking altogether. Europeans, as of this year, enjoy a "right to be forgotten" which requires companies there to scrub their systems of any digital records at a consumer's request.? This option should be available, and realistic, for U.S. consumers, too.

Privacy expert Ieuan Jolly tried to take an even-handed approach when talking about the data brokerage industry this week, beginning his analysis with an "On the one hand," statement, saying that data analytics helps companies meeting consumers' needs. But "the other hand" doesn't really seem to offer much of a balanced scale.? "On the other hand, few consumers know the identity of these invisible puppet masters that are collecting masses of data about them."

If they knew, they'd demand better. So I'm telling you: Instagram said it would sell your pictures, and you rioted. Data brokers, these digital puppet masters, sell your whole lives. Isn't that at least worth an e-mail to Congress?

UK PM says troops to quit Afghanistan

Around 3800 British troops will be withdrawn from Afghanistan during 2013, British Prime Minister David Cameron says.

Cameron told parliament that the strength of the UK force will be reduced from 9000 to 5200 by the end of next year.

The partial withdrawal paves the way for the final removal of the bulk of British personnel from Afghanistan as planned by the end of 2014, Cameron said.

PAKSAT-1R has a design life of 15 years and will provide TV broadcasting, Internet and data communication services across South and Central Asia, Eastern Europe, East Africa and the Far East. This satellite now enables extending of communication services to all areas of Pakistan. The National Command Authority (NCA) recently approved Pakistan?s Space Programme 2040 during its meeting at the Strategic Plans Division (SPD) on 14 July 2011. Earlier, the contract for the development of PAKSAT-1R was signed between Pakistan Space and Upper Atmosphere Research Commission (SUPARCO) and China Great Wall Industry Corporation (CGWIC) in October 2008 during the visit of the President of Pakistan to China. President: Mandela may spend more days in hospital

JOHANNESBURG (AP) ? Anti-apartheid leader Nelson Mandela could spend a few more days in the hospital for care, South Africa's president said Tuesday, as the 94-year-old spent an eleventh day under doctors' care after suffering from a lung infection and undergoing surgery.

President Jacob Zuma said Mandela's age meant the politician needed "extraordinary care" from medical professionals. However, he tried to assure a public increasingly worried about the health of a man many view as a living embodiment of the nation's hopes after it abandoned white racist rule and became a full democracy.

Doctors "say there is no crisis, but add that they are in no hurry to send him home just yet until they are satisfied that he has made sufficient progress," Zuma said in a statement issued by his office. "We urge the public to continue supporting (Mandela), but at the same time to understand that he is 94 years old and needs extraordinary care. If he spends more days in hospital, it is because that necessary care is being provided."

The statement also said presidential spokesman Mac Maharaj visited Mandela on Tuesday, who immediately asked: "Mac, what are you doing here?"

Tuesday marked Mandela's eleventh day in hospital. South Africa's government has said the 94-year-old Mandela was admitted Dec. 8 to a hospital in the country's capital, Pretoria, initially for medical tests. It then said Mandela had a recurrent lung infection, a chronic problem as he contracted tuberculosis while in prison and had an acute respiratory infection in 2011.

On Saturday, the government said Mandela underwent endoscopic surgery to remove gallstones. That's a procedure in which a patient usually receives sedatives and an anesthetic to allow a surgeon to put an endoscope down the throat, authorities say. The surgeon then removes the gallstones, which are small, crystal-like masses that can cause a person tremendous pain.

Mandela, a leader in the anti-apartheid liberation movement called the African National Congress, served 27 years in prison before being released. He later became the nation's first democratically elected president in 1994 under the ANC's banner. He served one five-year term before largely retiring from public life.

In the time since, Mandela has lived a private life, although he actively campaigned against HIV/AIDS. He last made a public appearance when his country hosted the 2010 World Cup soccer tournament. The Nobel laureate has also grown more frail in recent years, with his grip on politics in the nation ever slackening.

Syrian vice president says army can't win

BEIRUT (AP) ? Syria's vice president has acknowledged that the army cannot defeat the rebel forces trying to topple the regime and called for a negotiated settlement to save the country from ruin.

The rare, candid comments by Farouk al-Sharaa, a longtime close aide to President Bashar Assad's family, suggested his embattled regime may be contemplating an exit strategy as rebel forces move closer to the capital Damascus. He spoke in an interview published Monday by Lebanese newspaper Al-Akhbar.

"I don't see that what the security forces and army units are doing will lead to a definitive victory," al-Sharaa was quoted as saying in the interview conducted in Damascus.

"All these opposition forces can only conclude the battle to topple the regime if their goal is to push the country into chaos and a cycle of violence that has no end," he added.

Al-Sharaa pushed for a negotiated political settlement that includes the formation of a national unity government with wide jurisdiction.

His comments coincided with a step-by-step peace plan for Syria outlined by Iranian officials on Sunday. It would be capped by Syrian elections that presumably could usher in a new leader in Damascus.

Tehran is Assad's closest and perhaps only remaining regional ally and the initiative suggests its embrace of the Syrian president could be cooling.

The initiative ? while almost certain to be rejected by Syrian rebel factions ? marks one of the clearest signals yet that Iran's leadership is looking to hedge its bets and remain a player in Syrian affairs if Assad is toppled.

It was unclear whether al-Sharaa's comments were timed to coordinate with the Iranian initiative.

"Despite his rhetoric, Bashar Assad may now be contemplating an exit strategy -- one which would allow him to seek refuge abroad with his neck intact," said Anthony Skinner, an analyst at Maplecroft, a British risk analysis company.

Al-Sharaa, 73, a longtime loyalist to the Assad family, has been a controversial figure since the start of the uprising.

He appeared in public in late August for the first time in weeks, ending repeated rumors that he had defected. The regime has suffered a string of prominent defections in recent months, though Assad's inner circle and military have largely kept their cohesive stance behind him. Assad and his inner circle are predominantly Alawites, a minority sect that is an offshoot of Shiite Islam. The opposition is dominated by the majority Sunni Muslims.

Early on in the uprising, the Syrian president delegated to al-Sharaa, a skilled diplomat, responsibility for holding a dialogue with the opposition. A Sunni from the southern town of Daraa, birthplace of the Syrian uprising, al-Sharaa's silence since the start of the uprising made him a prime candidate for rumors that he broke with the regime.

His comments after a long silence suggest he may be have been given a green light to sound out readiness for a political settlement.

Syrian rebels have made significant tactical advances in the past weeks, capturing air bases and military installations near Syria's largest city of Aleppo and in the capital Damascus. On Sunday, an Islamist faction took an infantry base in Aleppo, a second army base that was captured from the troops in the northern city in a week.

Also, Western nations are talking of stepped up aid to the rebels. And there were mixed messages last week from Assad's key international ally Russia, which tried to backpedal after a top diplomat said Assad is losing control of his country.

Al-Sharaa offered an unusually bleak public assessment of the civil war and even criticized how the government has handled the crisis.

"Every day that passes, the military and political solution gets more elusive," he said. "We need to be in a position to defend Syria. We are not in a battle for an individual or a regime."

In October, the Turkish leadership appeared to be making a diplomatic push to promote al-Sharaa as a possible figure to head a transitional administration to end the conflict.

"No one knows the system better than Farouk al-Sharaa," Turkish Foreign Minister Ahmet Davutoglu said at the time, adding that al-Sharaa has not been involved in the violence and massacres.

The Syrian opposition is deeply fragmented, and various factions would likely disagree on whether they would accept him to lead a transitional government. Al-Sharaa, in the interview, said he was not seeking such a role.

Violence across many parts of the country, including a Palestinian refugee camp on the outskirts of Damascus, flared again on Monday.

U.S. State Department spokesperson Victoria Nuland said the United States is "deeply concerned" by reports that dozens of civilians were killed or wounded in Yarmouk camp as a result of aerial bombardment and fighting between Syrian government forces and armed opposition.

At least eight people were killed in the airstrike on Yarmouk Sunday, according to Syrian activists.

Ninette Kelley, a Lebanon representative for the U.N. refugee agency UNHCR, said 22 buses carrying 100 Palestinian families from Yarmouk crossed into Lebanon in the past 24 hours, fleeing the violence.

Syrian Foreign Minister Walid al-Moallem claimed the clashes at the camp were triggered by the al-Qaida-affiliated Jabhat al-Nusra group, which was designated by the Obama administration a terrorist organization last week. He warned Palestinians inside the camp not to harbor terrorist fighters.

Syria's official news agency SANA said Moallem's remarks were made during a telephone call with U.N. Secretary-General Ban Ki-moon.

Separately, Italy's government said three workers at a Syrian steel plant, including an Italian, have been kidnapped.

The Foreign Ministry did not say where or when the kidnappings occurred. But it said the plant is located in the regime stronghold of Latakia city on Syria's Mediterranean coast. The ministry statement said the two workers kidnapped with the Italian were citizens of other countries, but did not identify them further.

Sky TG24 TV in Italy reported the other two hostages are Russians, but there was no immediate confirmation of that.

Money launderers bribed Spanish cops Chinese politicians

.- The money laundering network allegedly headed by Chinese national Gao Ping facilitated its operations with bribes to customs agents in Spain and politicians in China, Spanish police say.

EFE gained access to a May 2012 police report on "Operation Emperor" that included transcripts of intercepted telephone conversations among the suspects.

In a conversation involving Wei Lin, who is considered to be one of Gao's lieutenants, the parties refer to customs workers and show, police say, "that the purchasing of favours is and has been a habitual practice in the recent past" in their commercial operations.

The criminal outfit also bribed Chinese officials, according to what was gleaned from another conversation between a man of Chinese origin and Gao.

In that conversation, the man asks Gao to contact politicians in the province of Guang Xi so that they would give the "green light" for a real estate project, in exchange for which he offered Gao part of the stock in the venture.

Gao answers that he will help the man find the "proper politician" because he knows "many in the area".

A Spanish judge Nov 29 ordered the release from jail of Gao and 11 other suspects due to a judicial error in extending the time they were under arrest.

The 12 suspects were ordered to surrender their passports and check-in with the court daily in person.

Operation Emperor lasted two years and is one of the largest dealing with laundering of assets and fiscal crime ever carried out in Spain.

Among those arrested are public officials, businessmen, politicians and even a well-known Spanish porn actor.

The popularity of online shopping has been seen by many as a threat to the existence of traditional shopping. Their argument is based on the recent widespread closure of established brands like Sears and Best Buy. But can we really say that traditional shopping is dying?

This figure is expected to rise in 2012 and subsequent years. What then is the implication of all these to physical retailing?

Since many people now decide to shop over the internet due to the benefits derivable in doing so like convenience, easy access to market, price comparison, avoidance of crowd, better prices and so on, can we really say that that the traditional way of shopping is being threatened? In my own opinion, technology will enhance the traditional method and not kill it. The popularity of video cassette recorder (VCR) in the 80?s was seen by many as a threat to movie theaters then, but today, the film industry has been strengthened by the new technology and the demand for films has increased.

Moreover, when the email became very popular, a lot people thought that the traditional postal service would die, but today, it is waxing stronger. The point I?m driving at here is that, if traditional shop owners can embrace new technology, shopping on the internet would not be a threat to them but a blessing.

If physical retailers can monitor shopping behavior through the use of Wi-Fi signals from smartphones, they would be able to compete favorably in a fast-changing world. They can use technology to measure loyalty, make better staffing decisions, improve store layouts, reduce wait times in checkout lines and so on. Online shopping has come to stay but it is not a threat to traditional shopping because many consumers still value some of its features like quality of customer service, the experience of trying and buying products, how products are displayed and so on.

Schizophrenia linked to social inequality

Dec. 14, 2012 ? Higher rates of schizophrenia in urban areas can be attributed to increased deprivation, increased population density and an increase in inequality within a neighbourhood, new research reveals. The research, led by the University of Cambridge in collaboration with Queen Mary University of London, was published today in the journal Schizophrenia Bulletin.

Dr James Kirkbride, lead author of the study from the University of Cambridge, said: "Although we already know that schizophrenia tends to be elevated in more urban communities, it was unclear why. Our research suggests that more densely populated, more deprived and less equal communities experience higher rates of schizophrenia and other similar disorders. This is important because other research has shown that many health and social outcomes also tend to be optimal when societies are more equal."

The scientists used data from a large population-based incidence study (the East London first-episode psychosis study directed by Professor Jeremy Coid at the East London NHS Foundation Trust and Queen Mary, University of London) conducted in three neighbouring inner city, ethnically diverse boroughs in East London: City & Hackney, Newham, and Tower Hamlets.

427 people aged 18-64 years old were included in the study, all of whom experienced a first episode of psychotic disorder in East London between 1996 and 2000. The researchers assessed their social environment through measures of the neighbourhood in which they lived at the time they first presented to mental health services because of a psychotic disorder. Using the 2001 census, they estimated the population aged 18-64 years old in each neighbourhood, and then compared the incidence rate between neighbourhoods.

The incidence of schizophrenia (and other similar disorders where hallucinations and delusions are the dominant feature) still showed variation between neighbourhoods after taking into account age, sex, ethnicity and social class. Three environmental factors predicted risk of schizophrenia -- increased deprivation (which includes employment, income, education and crime) increased population density, and an increase in inequality (the gap between the rich and poor).

Results from the study suggested that a percentage point increase in either neighbourhood inequality or deprivation was associated with an increase in the incidence of schizophrenia and other similar disorders of around 4%.

Dr Kirkbride added: "Our research adds to a wider and growing body of evidence that inequality seems to be important in affecting many health outcomes, now possibly including serious mental illness. Our data seems to suggest that both absolute and relative levels of deprivation predict the incidence of schizophrenia.

"East London has changed substantially over recent years, not least because of the Olympic regeneration. It would be interesting to repeat this work in the region to see if the same patterns were found."

The study also found that risk of schizophrenia in some migrant groups might depend on the ethnic composition of their neighbourhood. For black African people, the study found that rates tended to be lower in neighbourhoods where there were a greater proportion of other people of the same background. By contrast, rates of schizophrenia were lower for the black Caribbean group when they lived in more ethnically-integrated neighbourhoods. These findings support the possibility that the socio-cultural composition of our environment could positively or negatively influence risk of schizophrenia and other similar disorders.

Dr John Williams, Head of Neuroscience and Mental Health at the Wellcome Trust said: "This research reminds us that we must understand the complex societal factors as well as the neural mechanisms that underpin the onset of mental illness, if we are to develop appropriate interventions."
